Health Care Proxies

Unlike a Power of Attorney, which can be used both while you are well and should you become incapacitated, a health care proxy only becomes effective when you are unable to participate in your medical care.  

 

A health care proxy allows you to name the person you want to make medical decisions for you.  The person you list on your proxy is your health care agent and has access to your physicians, your medical records and is able to speak with health care providers about your condition.
